Incidental finding in a patient undergoing CTPA for suspected pulmonary embolus. No prior history of hepatectomy.

The left hepatic lobe is largely absent with a small amount of remnant tissue noted adjacent to the falciform ligament.
The left hepatic artery is not identified.
The left portal vein is also not appreciated (despite scan not being the portal venous phase).
The right hepatic lobe and caudate lobe demonstrate mild compensatory hypertrophy.
Case Discussion
Incidental finding.
Absence of left hepatic vascular structures confirms developmental anomaly rather than atrophy of the left lobe.
