Bilateral central bronchiectasis with broncoarterial ratio greater than 3 (normal range < 1).
Left 6th segmentar and subsegmentar bronchi are filled with high attenuation mucus plug (HAM) with a typical "finger in glove" sign.
Several centrilobular micronodules in the 6th segment of the left lung are consistent with the "tree in bud" sign.
No pulmonary embolism.
Case Discussion
Central bronchiectasis with hyperdense mucus plug in the bronchi are highly suggestive for allergic bronchopulmonary aspergillosis (ABPA).
