Amiodarone deposition in the liver

Presentation

Cardiac patient on amiodarone for the last 5 years. Otherwise asymptomatic.

Patient Data

Age: 85 years
Gender: Female

Diffusely increased hepatic attenuation (average density=90 HU) compared to the normal baseline attenuation values of the spleen.

Case Discussion

Amiodarone, a commonly prescribed antiarrhythmic medication, contains a significant iodine component ( 40% of its molecular weight ). Amiodarone undergoes primary metabolism in the liver, where it is converted to an active metabolite known as desethylamiodarone. Gradual and progressive  accumulation of amiodarone and its metabolite desethylamiodarone contributes to increased liver density, primarily attributed to its iodine content.
