Presentation

Right eye pain and swelling.

Age: 40 years
Gender: Female

CT of the maxillofacial structures with intravenous contrast demonstrates abrupt non-opacification of the right angular vein as it approaches an area of soft tissue inflammation, consistent with periorbital and preseptal cellulitis. The contralateral angular vein is appropriate.

Case Discussion

This is a case of angular vein thrombosis in the setting of preseptal cellulitis.

The patient presented to the emergency department (ED) with swelling around the right eye and was found to have thrombosis of the angular vein as it approached the area of inflammation. Otolaryngology evaluated the patient in the ED and she was treated conservatively with antibiotic therapy. She was discharged and has been in good health since.
