Anterior and posterior cruciate ligaments mucoid degeneration with ganglion cyst

Presentation

Pain in knee

Patient Data

Age: 40 years
Gender: Female

Diffuse thickening of anterior cruciate ligament is seen with maintained
continuity.

Large lobulated cystic lesion measuring 20 x 15 x 23 mm (AP x TD x CC) is seen along femoral attachment of anterior cruciate ligament. It is causing scalloping, erosions in adjacent medial aspect lateral femoral condyle with surrounding marrow edema.

Thickening of posterior cruciate ligament is also seen with maintained continuity.

Radial tear of junction of posterior horn and root of medial meniscus is seen with
meniscal extrusion.

Linear hyperintensity is seen in anterior and posterior horns of lateral meniscus
not intersecting with articular margins.

Other degenerative changes like chondromalacia patella, loss of tibiofemoral articular cartilage, marginal osteophytes and subchondral erosions.

Case Discussion

Findings are consistent with mucoid degeneration of anterior and posterior cruciate ligaments with a large ganglion cyst along femoral attachment on anterior cruciate ligament.

Mucoid degeneration of anterior cruciate ligament gives a 'celery stalk' and that of posterior cruciate ligament a 'tram track' appearance.
