Anterior cruciate ligament mucoid degeneration

Presentation

History of chronic knee pain. No traumatic context.

Patient Data

Age: 55 years
Gender: Female

The ACL appears diffusely thickened with hypersignal in the PD fat sat sequence alternating with normal hyposignal fibers showing the "Celery stalk sign". No ACL fibers disruption.

Small Baker cyst and minimal joint effusion also to be noted.

Case Discussion

Mucoid degeneration is a cause of increased signal in the ACL. The celery stalk appearance, the non traumatic context and the absence of associated lesions (normal aspect of the menisci and of the bone signal) are key for ruling out other differentials such as a partial tear.
