Avulsion of the right anterior inferior iliac spine, with fragmentation and displacement.
Incidental bone island on left femoral neck.
Case Discussion
This is a common pelvic avulsion fracture, that occurs due to sudden forceful hip extension while contraction of the straight head of the rectus femoris muscle that originates from the anterior inferior iliac spine. Management is usually conservative.
Of note, radiographs show a false negative John Thomas sign, a joke sign that has no scientific basis.
