Anterior myelomeningocele - thoracic spine

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Weakness of the lower limbs in a patient with neglected thoracic kyphoscoliosis. MRI for evaluation.

Patient Data

Age: 50 years
Gender: Male

The MRI sequences demonstrate:

  • upper thoracic kyphoscoliosis of left-sided convexity

  • block vertebrae at several levels

  • CSF-filled sac in continuity with the thecal sac extending to the posterior mediastinum through a vertebral defect. Thinning with tortuosity of the adjacent segment of the spinal cord bulging through the vertebral defect well-demonstrated on sagittal STIR sequence (zoomed sequence)

Case Discussion

MRI features of upper thoracic kyphoscoliosis with block vertebrae at several levels and anterior myelomeningocele.

Anterior myelomeningocele is rarely seen in the thoracic region and only few cases were reported in the literature 1,2.
