Case contributed by Ahmed Elhusseiny
Diagnosis almost certain

Presentation

Anterior knee pain

Patient Data

Age: 30 years
Gender: Male

Enlarged and edematous anterior suprapatellar fat pad (14 mm AP diameter) with convex border. Mild elevated signal of the quadriceps tendon is noted with mild bone marrow edema of the superior patellar pole. Fluid seen within semimembranosus bursa.

Case Discussion

The definitive diagnosis of anterior suprapatellar fat pad impingement hinges on clinical presentation, not solely on imaging findings. Asymptomatic fat pad enlargement is a common finding and should always be correlated with clinical presentation of anterior knee pain.
