Presentation
Known primary malignancy in neck, MRI brain to rule out metastasis.
Patient Data
A prominent cystic lesion is seen in left anterior temporal lobe with perilesional edema. No obvious solid component, internal septations, hemorrhage or enhancement on post-contrast study is seen.
No obvious elevated choline or reduced NAA seen on spectroscopy.
No obvious enhancing lesion seen elsewhere in brain parenchyma.
Bilateral mastoiditis is seen.
Case Discussion
Above findings suggest likelihood of a typical anterior temporal lobe perivascular space.
Lack of any obvious solid component, internal septations, hemorrhage or enhancement on post-contrast study and absence of any other lesion in rest of brain parenchyma is further against the likelihood of this lesion being metastasis.
