Case contributed by Craig Hacking
Diagnosis certain

Presentation

Echo shows aortic valve mass.

Patient Data

Age: 60 years
Gender: Female
ct

ECG -gated CTCA protocol.

Total coronary artery calcium score is 0.

There is a 9 x 10 x 12 mm irregular hypodense mass seen at the aortic root, abutting the noncoronary sinus and valve cusp.

Right dominant circulation.

Left Main: No significant disease

LAD/diagonal: No significant disease. D1 is a small vessel. D2 is normal.

RCA/acute marginal/PDA: No significant disease.

Circumflex/obtuse marginal: No significant disease.

No mediastinal adenopathy or pulmonary lesion in the imaged volume.

Impression

  1. Calcium score is zero.

  2. No evidence of obstructive coronary artery disease.

  3. The known aortic valve mass is redemonstrated.

Case Discussion

The patient underwent sternotomy and resection of the lesion. The valve was normal in appearance and function after lesion removal, so the surgeon elected not to proceed with valve replacement. The patient had an uneventful post operative recovery.

Histology confirmed papillary fibroelastoma.
