There are patchy areas of diffusion restriction in the bilateral paramedian thalamic and midbrain with corresponding T2W/FLAIR hyperintensity, suggestive of acute infarct. The midbrain V sign is seen on the DWI and FLAIR images.
Case Discussion
The clinical details and MRI findings are suggestive of Artery of Percheron territory infarct. The close differential includes top of basilar artery syndrome.
