Atypical femoral fracture

Patient Data

Age: 75 years
Gender: Female
x-ray

There is an acute, horizontally displaced fracture in the sub-trochanteric left femur. Both hips are enlocated and the pelvis is intact.

Case Discussion

Horizontal subtrochanteric fractures with a history of low-energy trauma are strongly suggestive of an atypical femoral fracture due to bisphosphonate use (which was confirmed).

The contralateral femoral shaft should be X-rayed for signs of incomplete insufficiency fracture such as focal periosteal new bone formation. This may be treated prophylactically with an intramedullary nail.
