Base of coracoid process fracture

Diagnosis probable

Presentation

Works as a butcher. Presented with persistent shoulder pain with no response to conservative treatment. No clear trauma history given.

Patient Data

Age: 55 years
Gender: Female

Fracture of coracoid process of scapula near its base with mild anterior angulation. Diffuse bone marrow edema of the fractured fragment.

Bone marrow edema is also present around acromioclavicular joint - particularly in distal part of the clavicle. The coracoacromial ligaments are intact.

Case Discussion

In humeral metaphysis there are two small foci of intermediate T2 signal with chemical shift artifact of unknown significance (the study is limited by lack of intravenous contrast). No trauma history raises suspicion of possible pathological fracture, though no clear tumor mass is visible in scapula. The further diagnostic work-up is required.
