Presentation

Bilateral heel pain.

Patient Data

Age: 50 years
Gender: Male

Both ankle joints

mri

Fusiform swelling of both tendo-Achilles, reflecting chronic tendinopathy.

This is associated with (5.5 cm) gap at (5.2 cm) distance from left calcaneal insertion.

This is associated with (4.7 cm) gap at (4.5 cm) distance from right calcaneal insertion.

Case Discussion

Features are consistent with complete non-insertional tear of both tendo-Achilles top of chronic tendinopathy. According to Kuwada classification, both sides are considered grade III tear which require tendon/synthetic graft.
