Bilateral soleal line - tug lesion

Presentation

Knee pain.

Patient Data

Age: 55 years
Gender: Female

Oblique sclerotic lines in the proximal metaphysis of both tibia bones inferring soleal lines.

Subchondral sclerosis and joint space narrowing are evident on the medial side of both knees. In addition, degenerative changes are seen as sharpening in the tibial spines and the posterior aspect of the patella of both knees. There is also evidence of enthesopathy, including bony projections in the anterior aspect of both patellar.

Case Discussion

This case demonstrates prominent soleal lines, also known as tug lesions, a bilateral and symmetrical anatomical variant in the attachment site of the soleal muscle at the posterior aspect of the tibia.

It should not be confused with pathological heterotopic bone or periosteal elevation and is placed as a do-not-touch lesion.
