Biliary cystadenoma with bile duct communication demonstrated on post ERCP CT study

Diagnosis probable

Presentation

Post ERCP abdominal pain.

Patient Data

Age: 60 years
Gender: Female

Post-ERCP contrast in the biliary tree is noted at CBD, gallbladder, and also into the cystic lesion of segment 4 of the liver, which has incomplete internal septations. There is pneumobilia.

No pneumoperitoneum or pancreatitis.

MRI of the Liver with contrast prior to the ERCP demonstrated a well-defined multiloculated cystic lesion in segment 4 of the liver. It shows thin rim and septal enhancement. No enhancing solid nodular component within. No evidence of restricted diffusion or surrounding parenchymal edema.

There are small gallstones and CBD stones without any significant biliary dilatation.

Case Discussion

Current study demonstrates the communication of biliary cystadenoma with the biliary tree.

Another way of facilitating the diagnosis is the use of liver-specific contrast media that demonstrate a communication between the lesion and the biliary tree, which helps in assessing the origin of a cystic tumor from the biliary ducts.
