Bipartite hallux sesamoid diastasis

Presentation

Medial, plantar foot pain

Patient Data

Age: 40 years
Gender: Female

Both medial hallux sesamoids appear bipartite. The left medial hallux sesamoid synchondrosis appears widened compared to the right. The edges appear sclerotic and slightly irregular. 

Bipartite medial hallux sesamoid with medially widening of an edematous synchondrosis. The proximal fragment appears edematous with low T1 signal. The distal fragment demonstrates normal signal. No fragmentation.

Medial/proximal aspect first metatarsophalangeal capsular injury. 

Post-operative

x-ray

Internal fixation of the medial hallux sesamoid with a single screw. Synchondrosis widening is reduced. 

Case Discussion

Diastasis of a hallux sesamoid is a rare injury. Clinically, the differential diagnosis is turf toe. Radiologically, the differential diagnosis includes a fractured unipartite hallux sesamoid.
