Bipartite patella - type I

Presentation

Pain following injury.

Patient Data

Age: 10 years
Gender: Male

Elongation of the patella is seen with a well-defined bone fragment along the inferior pole of the patella. Associated marrow edema is seen. Edema is also seen in the infrapatellar fat pad. theThe patellar articular cartilage is continuous along with the above-mentioned bone fragment.

Low to intermediate grade tear of mid-segment of the anterior cruciate ligament is seen.

Case Discussion

The MRI findings are consistent with a type I bipartite patella.

The differential diagnosis would be Sinding Larson Johansson disease. However, this case the bone fragment along the inferior pole of patella is well defined with smooth contours and no obvious fragmentation; thus favoring bipartite patella. The soft tissue and marrow edema can be explained by a history of injury
