Blocked left nasolacrimal duct

Presentation

Left eye epiphora (excessive tearing).

Patient Data

Age: 40 years
Gender: Male
Fluoroscopy

Opacification of left nasolacrimal duct proximal segment up to mid-segment. No filling defect.

Non-opacification of distal segment of left nasolacrimal duct.

Case Discussion

5 ml of Ultravist-370 (low osmolar contrast medium, LOCM) was used.

27G Sterimedix needle inserted into the left canaliculus at the medial canthus. The needle tip is inserted too deep into the nasolacrimal sac instead parked within the inferior canaliculus.

LOCM was injected via needle and digital subtraction angiography (DSA) scan was done.

The contrast opacified the superior and inferior canaliculus through reflux because of obstruction of the left nasolacrimal duct.
