Case contributed by Mohammad Taghi Niknejad
Diagnosis almost certain

Presentation

Left breast palpable mass and nipple retraction on physical exam.

Patient Data

Age: 50 years
Gender: Female
mammography

A large hyperdense mass with irregular and spiculated margin and fine internal calcification foci is noted in the upper central part of the left breast that causes surrounding parenchymal distortion and nipple retraction. (BI-RADS 5)

Scattered punctate calcification foci as a benign finding are seen in both breasts. (BI-RADS 2)

ultrasound

A large hetero-echoic mass with irregular and spiculated margin causes surrounding parenchymal distortion. (BI-RADS 5)

Case Discussion

Ultrasound-guided core needle biopsy is performed for the left breast's highly suspicious mass, and histopathology evaluation confirms invasive breast carcinoma of no special type.

As a rule, a breast mass with irregular and spiculated margins that contains fine internal calcification foci and causes surrounding parenchymal distortion and/or nipple retraction is highly suspicious for malignancy (BI-RADS 5), and further biopsy is needed for histopathology and IHC evaluations.
