Bucket handle tear - lateral meniscus

Diagnosis almost certain

Presentation

Twisting injury of the knee.

Patient Data

Age: 18 years
Gender: Male

Bucket handle tear of the lateral meniscus with displacement of the meniscal fragment of the posterior horn medially to the intercondylar notch adjacent to posterior cruciate ligament. Part of the posterior horn is flipped anteriorly.

Anterior displacement of the anterior horn of lateral meniscus.

Moderate knee joint effusion.

Intact medial meniscus with no tears.

Intrasubstance high T2 signal the lateral collateral ligament, reflecting grade II injury.

Case Discussion

MRI appearance of bucket handle tear of the lateral meniscus involving the posterior horn that is displaced at the intercondylar notch. Bucket handle tears are more common involving the medial meniscus. Part of the posterior horn is flipped anteriorly giving a double anterior horn sign or double delta sign.
