Thinning of the lower anterior uterine wall is noted, with focal triangular discontinuity of the myometrium and a small amount of T1 high signal intensity fluid/blood.

The accumulation of blood in the cesarean scar niche may serve as a source of dysfunctional uterine bleeding, explaining the vaginal spotting observed after the routine menstrual cycle in this case.
