A biventricular cardiac conduction device with leads in the right atrium, right ventricle and cardiac vein via the coronary sinus.
Cardiomegaly.

Biventricular cardiac conduction devices are used for cardiac resynchronisation therapy. Dessynchrony is characterized by a wide QRS-complex on ECG.
This device has a defibrillator function apparent from the shocking coil of the right ventricular lead.
Ideally the left ventricular lead should be positioned in a vein over the area with the most delayed intrinsic activation, avoiding areas of infarction, Cardiac venous anatomy is variable.
