Cavernoma

Diagnosis almost certain

Presentation

Frontal seizures.

Patient Data

Age: 60 years
Gender: Female
mri

A lesion extends from the pial surface of the inferior frontal gyrus (involving the posterior part of pars triangularis) to the ependymal surface of the frontal horn of the lateral ventricle. It has a prominent peripheral hemosiderin staining and signal loss on susceptibility-weighted imaging. Some intrinsic high T1 signal and minimal central enhancement. No surrounding edema. No abnormal flow voids or solid enhancement. No intraventricular hemosiderin staining or superficial siderosis was seen elsewhere. The remainder of the brain is unremarkable in appearance.

Conclusion:

Although unusual in extent, the features are characteristic of a cavernoma.
