Cephalization of pulmonary veins in pulmonary edema

Diagnosis probable

Presentation

Dyspnea.

Patient Data

Age: 60 years
Gender: Male

Interval enlargement of the heart with distension of the pulmonary vasculature with dilated upper lobe pulmonary veins, suggestive of early cardiogenic pulmonary edema.

Case Discussion

Normally, because of gravity, there is less blood flow into the upper lobes and the upper lobe pulmonary veins are small in caliber. But in left heart failure, left atrial pressure increases, resulting in distension of the upper lobe pulmonary vessels. Upper lobe pulmonary venous diversion or cephalization of the pulmonary veins is an early sign of left heart failure.
