Presentation

Chronic left facial dysesthesia.

Patient Data

Age: 40 years
Gender: Female

In the left cerebellopontine angle, an extra-axial and heterogeneous lesion is observed, with irregular morphology featuring lobulated and partially defined edges. It is hypointense to the grey matter on T1, hyperintense on T2, and heterogeneous, predominantly hypointense on FLAIR, with diffusion restriction, without present enhancement upon application of contrast medium.

This lesion obliterates the cistern of the cerebellopontine angle, displacing the cisternal portion of ipsilateral cranial nerves VII/VIII.

Case Discussion

Intracranial epidermoid cysts are congenital inclusion cysts. The most common location for epidermoid cysts is the cerebellopontine angle cistern (40% - 50%). Most are asymptomatic but may occasionally result in mass effect and cranial neuropathy.

On magnetic resonance imaging, epidermoid tumors typically have low signal intensity on T1-weighted images, high signal intensity on T2-weighted images, and no enhancement on gadolinium-enhanced images.
