Diagnosis certain

Presentation

Seizure.

Patient Data

Age: 30 years
Gender: Male

Evidence of serpiginous enhancing vascular tuft in the right frontal lobe with surrounding areas of gliosis and ex vacuo dilatation of the ipsilateral lateral ventricle.

Tracing of the arterial tree and venous system revealed multiple suppliers from the right middle cerebral artery and superior sagittal sinus as a draining vein.

Case Discussion

Cerebral arteriovenous malformation is a common cause of seizure in the young age group.

Ischemic changes result from long-standing vascular steal phenomena.

Engorged venous sinuses as evident in the superior sagittal sinus suggest an underlying high-flow shunt from the arterial to the venous system.
