Cerebral hydatid cyst

Presentation

Right-sided paraplegia.

Patient Data

Age: 5 years
Gender: Male

A large, well-defined, thin-walled, intra-axial unilocular cystic lesion is seen in the left cerebral hemisphere white matter (fronto-parieto-temporal lobes) with resultant mass effect, seen as effacement of the left lateral ventricle and cortical sulci, midline shift towards the right side and mild left uncal herniation. No evidence of post-contrast enhancement or surrounding edema. No internal septae or solid enhancing components. No evidence of calcification or hemorrhage. No diffusion restriction.

Mild prominence of bilateral temporal horns of the lateral ventricles.

Case Discussion

Considering the endemic area of the hydatid disease, the case was reported as a cerebral hydatid cyst, and the main differential diagnosis was a neuroglial cyst. The patient underwent surgery, which confirmed it was indeed a hydatid cyst.
