Cervical spine fracture with dislocation and locked facets

Presentation

High-speed car accident with collapse and lost consciousness.

Patient Data

Age: 25 years
Gender: Male

Post-traumatic RTA showing multiple unstable fractures as follow:

  • Fracture dislocation of C7/T1 with bilateral facetal dislocation (jumped facets) as well as marked anterolithesis (more than 75 %) with mostly evident cord injury for MRI assessment
  • C6 and C7 spinous processes avulsion fracture (clay shoveler fractures) with gapping and displacement.
  • C6 left, C7 bilateral, and T1 left transverse processes fractures.
  • T1 anterosuperior margin fracture (teardrop fracture)
  • Upper ribs fracture mainly at left side.
  • C7 para and pre-vertebral mild soft tissue thickening/hematoma.
  • Accidentally seen, left pneumothorax, pleural collection, and left upper lung lobe posterior segment consolidative patches at the limited visualized upper lung cuts.
  • ETT is also noted.

Case Discussion

Unstable C7-T1 fracture-dislocation with locked facets associated with three columns injuries and possible cord injury.
