Chalk stick fracture in ankylosing spondylitis

Presentation

Severe neck pain

Patient Data

Age: 55 years
Gender: Male

Fracture is seen involving disco-vertebral junction of C5-6 disc and superior endplate of C6 vertebral body with marrow edema in superior aspects of C6 vertebral body. Mild prevertebral edema / hematoma is also seen. 

Marrow edema is also seen involving posterior elements at few levels with edema involving intraspinous ligaments and posterior paraspinal muscles. 

Ossification of anterior and posterior longitudinal ligaments is seen with intradiscal calcifications and facet joint ankylosis at places in cervical spine.

Ossification of anterior longitudinal ligament is seen with multi-level costotransverse, costovertebral, is and possibly facet joint ankylosis in dorsal spine. Ossification / thickening of posterior longitudinal ligament is also seen mildly impinging on ventral thecal sac at places.

Ossification of anterior longitudinal ligament is seen with facet joint ankylosis at multiple levels in lumbar spine.

Partial ankylosis of bilateral sacroiliac joints is also seen .

Case Discussion

Above findings are consistent with ankylosing spondylitis with a chalk stick fracture at C5-C6 level.
