Presentation

Abdominal pain, vomiting, and acute pancreatitis.

Patient Data

Age: 5 years
Gender: Female

CBD is markedly dilated (about 18 mm) showing a tortuous course with a small amount of sludge/debris at its distal segment.

There is an anomalous pancreatico-biliary junction evidenced by long common channel between common bile duct and pancreatic duct outside the duodenum.

There is marked fusiform dilatation of the common bile duct (CBD) and common hepatic duct.
Multiple variable-sized filling defects of signal void seen within the distal portion of the CBD, representing stones.

Case Discussion

This case demonstrates features of choledochal cyst disease (consistent with type Ic) associated with distal CBD stones

It is important to measure the length of the common channel, in such case was about 14 mm, as it guides the surgical management.
