Presentation

Patient presents to the emergency department after a motor vehicle collision.

Patient Data

Age: 50 years
Gender: Female
x-ray

Three views of the right foot and three views of the right ankle were obtained.

  • acute fracture dislocation of the midtarsal joint with the talus dislocated from the navicular bone and the calcaneus dislocated from the cuboid; distracted subtalar joint; preserved ankle mortise

  • two bone fragments are noted proximal and posterior to the lateral malleolus which may represent bony fragment from the calcaneus

  • there is normal alignment of the Lisfranc joint at the second tarsometatarsal joint however the third and fourth tarsometatarsal joints are not well seen

  • there is cortical irregularity at the base of the third and fourth metatarsals which may represent nondisplaced fractures

  • also noted is a tiny bony fragment proximal to the fifth metatarsal which may represent a displaced fracture fragment

Case Discussion

Fracture-dislocation of the right midtarsal joint with lateral dislocation and displaced bone fragments lateral to posterior to the lateral malleolus compatible with Chopart fracture-dislocation.
