Chronic empyema

Diagnosis almost certain

Presentation

Reduced air entry on the right side, history of asbestosis exposure, ex-smoker 20/day, and known to have chronic airway disease.

Patient Data

Age: 70 years
Gender: Male
x-ray

There is sheet-like pleural calcification in the right lower zone parallel to the right lateral chest wall and associated with peaking and elevation of the right hemidiaphragm.

An old calcified node is seen in the right paratracheal region. The left lung is clear.

There is a thick rind of pleural thickening and calcification in the right hemithorax involving both visceral and parietal pleura. Extrapleural fat hypertrophy separates this from the chest wall.

There is volume loss of right hemithorax with crowding of ribs.
