Chronic obstructive pulmonary disease (COPD)

Case contributed by David Horvath
Diagnosis almost certain

Presentation

History of over 100 pack-years of cigarette smoking presents with symptoms of an acute COPD exacerbation.

Patient Data

Age: 75 years
Gender: Male

Enlarged right heart, flattened diaphragms, perihilar fullness, and diffuse interstitial opacification. Peripheral vascular markings are within normal limits.

Case Discussion

Most cases of COPD are caused by cigarette smoking. However, some cases are secondary to air pollution as seen in patients with occupational exposure to dust and fumes. A COPD exacerbation is most commonly caused by viruses. Viral etiology potentially explains why this patient has perihilar fullness and diffuse interstitial opacification.
