Presentation
Chronic right shoulder pain for several months, no direct injury reported. Sensitivity most pronounced along the course of the long head the biceps tendon (LHBT).
Patient Data
small, loculated fluid collection in the LHBT tendon sheath and pronounced hypervascularity of the thickened tendon sheath, best appreciated using microvascular flow imaging
Irregular, but continuous supraspinatus and subscapularis tendons, in line with chronic degenerative changes

acromioclavicular joint degenerative changes manifesting is capsular thickening
Case Discussion
Findings favor chronic tenosynovitis of the long head of the biceps tendon sheath.
