Presentation

Low back pain.

Patient Data

Age: 80 years
Gender: Female
mri

Mild (grade I) spondylolisthesis of L5 over S1 with diffuse posterior pseudo-disc bulge, partially encroaching upon exit foramina.

Biconcave compression fracture of L2 vertebral body with normal marrow signal and intact cortex, no retropulsion, impressive of old osteoporotic fracture.

Diffuse spondylodegenerative changes.

L5-S1 diffuse posterior disc bulge, partially encroaching upon exit foramina bilaterally.

Bilateral fracture pars interarticularis of L5 vertebra.

Mild (grade I) spondylolisthesis of L5 over S1 with diffuse posterior pseudo-disc bulge, partially encroaching upon exit foramina.

Old osteoporotic compression fracture of L2 vertebral body.  

Diffuse spondylodegenerative and osteoporotic changes.

L4-5 diffuse posterior disc bulge, partially encroaching upon exit foramina bilaterally.

Case Discussion

Fish vertebra (codfish vertebra) refers to the smooth biconcave appearance of vertebral body in sagittal plane in cross-sectional imaging or lateral view of lumbar radiograph. It can occur in one level or multiple levels. Commonly, it is seen with osteoprosis however it is not pathognomonic as it occurs in other diseases like sickle cell disease, renal osteodystrophy and thalassemia major.
