Common bile duct duplication - type Va

Presentation

Epigastric pain and mild jaundice. Prior history of cholecystectomy.

Patient Data

Age: 45 years
Gender: Female

Duplicated proximal extrahepatic bile ducts (without communicating channels) are seen reuniting into a single biliary drainage at the level of the head of the pancreas consistent with type Va duplication of common bile duct.

The cystic duct is seen draining to the right hepatic duct.

Gallbladder is resected.

An obstructive calculus measuring about 5 mm is seen in the common biliary channel just above the ampulla with subsequent mild dilatation of extrahepatic bile ducts.

Case Discussion

The patient went to have ERCP and removal of the obstructing calculus.

Modified Choi classification of common bile duct duplication is a widely used system for classifying extrahepatic duplication of the biliary tree which is a very rare anatomic variation.
