Complete left renal hypoplasia

Case contributed by James Cassels
Diagnosis probable

Presentation

Weight loss, anemia and rectal bleeding. Urgent request for possible cancer investigation.

Patient Data

Age: 80 years
Gender: Female

There is a complete left renal hypoplasia, with a 3.5mm upper pole calculus. The right kidney has undergone slight compensatory enlargement, and contains multiple simple cysts.

Additionally there are multiple uncomplicated gall stones.

There was no identifiable cause for the rectal bleeding or weight loss. Renal hypoplasia was completely incidental.

Case Discussion

This case is an example of renal hypoplasia affecting the entire left kidney (complete/global hypoplasia). This condition is often associated with increased incidence of urolithiasis, as seen here.

Given the size of the kidney, it is mostly likely an example of renal hypoplasia. However, unilateral renal atrophy is another differential. Without previous imaging it is difficult to differentiate.
