Presentation

Known case of congenital insensitivity to pain presented with progressive deformity of the right knee.

Patient Data

Age: 10 years
Gender: Male
x-ray

Right leg shows:

  • displaced fracture of the medial femoral condyle with secondary genu valgum and frying with sclerosis of the medial part of tibial epiphysis
  • green stick fracture of the mid-shaft of the tibia and fibula with evident periosteal reaction

Left knee shows a healing fracture of the lateral femoral condyle.

Six months later

x-ray

Follow up after 6 months revealed healed green stick fractures of tibia and fibula and non-union of the medial femoral condyle, with joint deformity.

Right knee joint

Displaced fracture of the medial femoral condyle with multiple bone fragments and sclerosis of fracture ends. Mild knee effusion. Secondary genu varum deformity of the knee. Irregular surface of medial part of tibial epiphysis.

Case Discussion

Multiple limb fractures with prominent periosteal reaction is a constant feature of congenital insensitivity to pain.
