Case contributed by Hidayatullah Hamidi
Diagnosis almost certain

Presentation

Memory disturbance with the inability to recognize family members. Disturbed gait.

Patient Data

Age: 75 years
Gender: Male
mri

Expansion of the splenium of the corpus callosum with low signal intensity on T1WI and heterogeneous high signal intensity on FLAIR with areas of central necrosis. Post-contrast images show peripheral irregular enhancement and sub-ependymal spread that extends to the medial part of the left temporal lobe. The mass shows relatively restricted diffusion and is surrounded by vasogenic edema. The lesion measures about 7 x 4 x 5.5 cm in TR, CC, and AP dimensions.

There is background age-related brain parenchymal involution.

Case Discussion

Findings are highly suggestive of glioblastoma involving the splenium of the corpus callosum. The heterogeneity of the signal of the lesion would make the differential of lymphoma to be less likely.
