Cortical amyloid deposition - Alzheimer disease

Presentation

Episodic memory, visuospatial and executive function impairment.

Patient Data

Age: 55 years
Gender: Female

Nuclear medicine

Evidence of abnormal cortical uptake of tracer throughout all lobes. Loss of the expected arboreal pattern of uptake, with abnormal "kissing hemispheres". Loss of grey-white matter contrast throughout the cerebrum.

On qualitative analysis, all structural SUVR values are abnormal. The average SUVR is 1.8 (normal <1.26). The centiloid value is 135 (abnormal >20).

The cerebral amyloid scan is considered positive, with a high burden of cortical amyloid distribution.

The abnormal study loses the physiological "arboreal" pattern of tracer distribution through the white matter due to abnormal accumulation in the grey matter of the cortex. The separation between the cerebral hemispheres is also significantly diminished.

Case Discussion

This case demonstrates significantly abnormal cortical amyloid deposition throughout the supratentorial brain, confirmed by both visual and quantitative analysis. This appearance would be consistent with Alzheimer disease.
