Crossed fused renal ectopia with bilateral hydronephrosis

Presentation

Bilateral leg edema. Outpatient ultrasound interrogation demonstrated "substantial left sided hydronephrosis".

Patient Data

Age: 55 years
Gender: Male

Large amount of fecal content in the non-dilated ascending, transverse, and descending colon.

Crossed fused left renal ectopia in right lower abdomen, with substantial bilateral hydronephrosis and hydroureter. The ureters are stretched laterally around the fecaloma and show distal tapering.

Case Discussion

Psychiatric patient with bilateral leg edema, more severe on the right. Creatinine levels rose from normal to 1.7 mg/dL. Treated with furosemide. "Substantial left-sided hydronephrosis" on outpatient ultrasonography. Referred to the emergency department.

CT stone protocol showed crossed fused left renal ectopia, probably type C (lump kidney), with substantial hydronephrosis. The ureters were stretched around a huge fecaloma - most probably the result of antipsychotic medication - and gradually tapered to a normal diameter.
