Cutaneous squamous cell carcinoma with nodal metastases

Presentation

Ulcerating left temporal cutaneous lesion.

Patient Data

Age: 80 years
Gender: Male

A large ulcerated left temporal cutaneous mass extends over an anterior-posterior diameter of 50mm. Subcutaneous extension to the level of the temporalis fascia. No bone destruction.

There are several enlarged suspicious intraparotid lymph nodes, the largest superiorly within the superficial lobe measuring 11 mm in short axis demonstrating an internal heterogeneous signal consistent with necrosis. Anterior to this 9 mm suspicious lymph node and further prominent lymph nodes at the left parotid tail. The left level 2a lymph node is also enlarged.

Case Discussion

This is a typical appearance of cutaneous squamous cell carcinoma of the scalp draining to the intraparotid lymph nodes (first-order nodal station for skin of scalp and face).
