Cytotoxic lesion of the corpus callosum

Diagnosis almost certain

Presentation

Sudden onset of blindness.

Patient Data

Age: 15 years
Gender: Female

T2 and FLAIR hyperintense and T1 hypointense well-circumscribed rounded/oval-shaped lesion in the midline within the substance of the splenium of the corpus callosum. It shows diffusion restriction and multiple SWI blooming foci with no post-contrast enhancement, mostly cytotoxic transient lesion of the corpus callosum.

Optic hydrops and indentation of the posterior sclera bilaterally are noted with partially empty sella, for further fundus examination to exclude the possibility of idiopathic intracranial hypertension.

Case Discussion

Imaging features are in keeping with cytotoxic lesions of the corpus callosum, many etiologies can be responsible for the occurrence of such lesion which is reversible and so follow up is recommended after one month or one week after neurological recovery.
