De Quervain tenosynovitis with retinacular ganglia

Presentation

Left wrist radial side pain - few days. No trauma.

Patient Data

Age: 70 years
Gender: Female

Hypoechoic retinacular thickening involving the first extensor compartment at the distal end of the radius. An anechoic thin-walled cyst (4 x 2 x 2 mm) in the thickened retinaculum. Intact tendons. No tendon sheath effusion.

Case Discussion

The patient presented with radial side wrist pain with thumb movements. Local soft tissue thickening was clinically visible along with a palpable small nodule in the region of 1st extensor compartment.

The ultrasound shows a thickened extensor retinaculum of the 1st extensor compartment known as De Quervain tenosynovitis. The clinically palpable nodule was a ganglion cyst in the thickened retinaculum which is an uncommon finding but not rare.
