Presentation

90 push-ups in 10 minutes as a penalty for losing a bet. About after twenty hours complained intense shoulders aches.

Patient Data

Age: 14 years
Gender: Male
ultrasound

Short axis to the humerus shows widespread, well-defined increased echogenicity of the medial, lateral, and long heads of triceps brachii and the anterior head of the deltoid muscle, bilaterally. The muscles are well-defined with diffuse and increased muscle size. Fasciae is not highly visible. No fibrillary disconnection or accumulation of fluid was seen.

Case Discussion

Delayed onset muscle soreness (DOMS) is a British athletics muscle injury classification (BAMIC) grade 1 muscle strain injury with peaks in muscle pain between 24-72 hours. DOMS on ultrasound shows a diffusely hyperechoic enlarged muscle and sometimes there may be increased flow on color and power Doppler imaging due to modest hyperemia.
