Dementia with Lewy bodies

Presentation

Cognitive impairment and delusions.

Patient Data

Age: 70 years
Gender: Female

The DAT SPECT splash images show abnormally reduced tracer uptake by the right striatum. There is also abnormally increased background cerebral uptake.

The corresponding FDG PET (axial and coronal reformats) shows decreased uptake in both temporal lobes, both parietal lobes (including the right precuneus), and notably in the right occipital cortex.

Case Discussion

The DAT scan appearances are positive for idiopathic Parkinson's or Parkinson's plus syndrome.

The temporoparietal hypometabolism and pre-cuneal involvement would suggest Alzheimer's dementia, but the right occipital hypometabolism combined with the abnormal DAT support the diagnosis of dementia with Lewy bodies.
