Diffuse uterine adenomyosis with ovarian and subperitoneal endometriosis

Presentation

Dysmenorrhea and chronic pelvic pain.

Patient Data

Age: 40 years
Gender: Female

The MRI sequences demonstrate:

  • enlarged uterus with diffuse thickening of the junctional zone endometrium-myometrium (16-20 mm) of intermediate signal on T1, and low signal on T2 in keeping with diffuse adenomyosis
  • well-defined right ovarian cystic mass measuring 56 x 50 x 46 mm of high signal on T1, T1 fat sat with T2 shading sign and no enhancement on postcontrast sequences in keeping with ovarian endometrioma
  •  thickening of the uterosacral ligaments mainly on the right as well as the torus uterinus with a small adjacent collection with a blood-fluid level in keeping with subperitoneal endometriosis
