L4-L5 disc degeneration with a disc herniation in the left subarticular zone. A small mass is noted posteroinferior to the disc herniation, compressing the adjacent nerve root. It displays an isosignal to the intervertebral disc on T1, T2, and STIR with peripheral enhancement on postcontrast sequences, suggestive of a free disc fragment that has no continuity with the native L4-L5 disc.
